What the Standard Panel Misses 

The typical annual bloodwork includes basic cholesterol numbers, blood sugar, and a general metabolic panel. These are useful but profoundly incomplete.

Here is what they routinely miss:

 Inflammation Markers

  • High Sensitivity CRP (hs-CRP) — the most accessible marker of systemic inflammation. Standard CRP misses low grade chronic inflammation entirely. The high sensitivity version catches it. As we established in Issue #4, chronic low grade inflammation is a primary driver of depression, cognitive decline, and cardiovascular disease

  • Homocysteine — an amino acid that accumulates when certain B vitamins are insufficient. Elevated levels are independently associated with heart disease, stroke, and Alzheimer's disease — yet it is almost never included in standard panels:

 

Metabolic Health

  • Fasting insulin — perhaps the single most important missing marker. Standard panels measure blood sugar but not insulin. You can have normal blood sugar and severely elevated insulin — the hallmark of early insulin resistance that may go undetected for a decade. As we covered in Issue #10, insulin resistance is the central thread of modern chronic disease

  • HbA1c — a three month average of blood sugar levels. Far more informative than a single fasting glucose reading

  • Triglyceride to HDL ratio — one of the most reliable indicators of insulin resistance and cardiovascular risk available from standard lipid panels — yet rarely calculated or discussed

 Hormonal & Nutritional

  • Vitamin D — functions more like a hormone than a vitamin. Deficiency is epidemic and linked to depression, immune dysfunction, insulin resistance, and increased cancer risk

  • Magnesium (RBC) — standard serum magnesium misses deficiency because the body maintains serum levels at the expense of cellular stores. Red blood cell magnesium is far more accurate — and deficiency affects over 300 enzymatic processes in the body

  • Thyroid panel — standard care tests TSH only. A complete picture requires Free T3, Free T4, and thyroid antibodies — particularly relevant for people with unexplained fatigue, weight gain, or depression

What You Can Do 

 Print this list and bring it to your next appointment — most of these tests are available through standard labs and many are covered by insurance when requested with appropriate clinical justification

  •  Ask specifically — doctors working within time-constrained appointments default to standard panels unless prompted. A simple "I'd like to add a few markers to my bloodwork this year" is often all it takes

  •  Track trends over time — a single result is a snapshot. The direction of travel across multiple tests over months and years is far more informative

  •  Know your optimal ranges — laboratory reference ranges are based on population averages, not optimal health. For example, Vitamin D sufficiency is listed as above 20 ng/mL by many labs — but most functional medicine practitioners target 50–70 ng/mL for optimal health
